COVID-19

Client: I understand that the novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend preventative measures including by not limited to social distancing, temperature checks, frequent hand washing, the use of hand sanitizer, and face coverings that cover both nose and mouth. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that I and/or my party may be exposed to or infected by COVID-19, and that such exposure or infection may result in personal injury, illness, disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 during travel may result from the actions, omissions, or negligence of myself and others. I agree to assume the aforementioned risks and accept sole responsibility for any effect on my party or myself, including, but not limited to, personal injury, disability, death, illness, damage, loss, liability, or expense of any kind, that I or my party may experience or incur in connection with this travel. 

 

I understand and accept that some amenities and activities in various destinations may be unavailable or altered in experience and can change at any given time. I acknowledge that for this reason, and other reasons not reasonably foreseeable at this time, my travel plans may be interrupted or cancelled by the supplier that is providing them, a government entity or other third party over which Destinations By Dani has no control. I further acknowledge that the supplier’s own cancellation, rebooking and refund policies, subject to any applicable law that is now or may later be in effect, will govern my rights and remedies, including my right to receive a refund, in such an event.

 

I also understand that a negative COVID-19 PCR or antigen test is required for air entry in some destinations, and for entry by air into the United States from all international destinations. I will make necessary arrangements to provide results of my test within the required time frame according to each destination. To that end, I also accept the risk of being required to quarantine in destination for 10-14 days should my test return a positive result. I will not knowingly travel to or from any destination with symptoms of COVID-19.

 

Moreover, I understand that should I elect to purchase travel insurance, the terms of the policy will dictate whether, and to what extent, coverage for any financial loss may exist under the circumstances. 

 

In terms of my travel arrangements with Destinations By Dani, I hereby agree to hold Destinations By Dani harmless and release it from any and all liability for any damages, including but not limited to monetary losses, I may incur as a result of such circumstances.
